Definitive Unilateral Cleft Rhinoplasty using Diced Cartilage Wrapped with Temporal Fascia: A Case Report

Abstract

|
A 24-year-old male patient showed a right unilateral cleft nasal deformity. Preoperative views demonstrated an asymmetric nose, depressed nasal tip, and flared right alar as well as a low dorsum. Computed tomography also revealed a deviated septum and inferior turbinate hypertrophy on the cleft side. He suffered from the secondary nasal deformity and difficult nasal breathing. Open rhinoplasty was performed via stair-step incision at the columella with a right-sided reverse-U incision. After right inferior turbinectomy, septoplasty was done to relieve the septal deviation whilethrough a section of septal cartilage was removed. Cartilage and fascial grafts were harvested from the right sixth rib and right temporal fascia. Composite reconstruction consisted of a deep foundation layer of spreader graft and columellar strutting overlaid with an esthetic layer of alar-contouring/shield graft and dorsal augmentation using diced cartilage in fascia. Postoperatively, the patient was very satisfied with improved nasal breathing and the esthetic nasal profile.
